Code 43 error

Hi my video card has stopped functioning. In device manager I am getting a Code 43 for my video card. The card is an EVGA GeForce 8500 GT and I'm running Windows Vista 32bit.

I found this link Explanation of error codes generated by Device Manager in Windows XP Professional saying

"One of the drivers controlling the device notified the operating system that the device failed in some manner.

I already have tried reinstalling the drivers, made no difference. Also, I had the latest drivers (from here: EVGA | Support | 8 Series Driver Download) when the problem occurred. I have also tried reverting to an older driver, no change.

When the problem occurred I was playing a game, then suddenly the graphics on the screen became all deformed and then the monitor stopped receiving any signal from the CPU (it went blank). At first I thought it was a problem with my monitor but when I restarted my computer and had a look in device manager I found error code 43 beside my Video Card

Could it be a problem with my OS not allowing the card to function properly?

When I restart my machine the video card is still being used except the graphics look like they would in safe mode. I mean I'm not using the motherboards graphic adapter since my monitor is still connected to the video cards port. So the video card still seems to be functioning.

I don't get any beeping noises either.

Also I’ve opened up my machine taken out the card, cleaned the pci express port and put the card back to see if the dust in the port might be causing the problem. On restart windows detected the card properly but it still game me a code 43 error.

Basically I'm wondering where the problem is and how I can fix it? Is it the card itself? The driver? The OS?
