Really depends on what operating system your mail server runs on and what MTA you're using. But an open relay is a mail server that is misconfigured in such a way that it will forward mail for anyone who asks. Obviously, most people in the world are not trustworthy, so open relays become spam relays in a matter of minutes. Can you be more specific as to what software we're dealing with here?

I won't ask why, but I will suggest that you start from scratch. NT4 hasn't been supported for a long time, and that box is probably compromised. It's best to start out with a fresh OS installation and migrate your users to a new mail server. If you want a secure, appliance-like mail server, look into SME Server. It's a free, easy-to-use and maintain Linux-based server. Best of all, it'll run perfectly well on your existing hardware.
